Nursing Program Application

This application is in addition to the college application. The admissions committee usually meets once a semester. Any applications received at least 2 weeks prior to the meeting are reviewed; applications received after are reviewed at the next meeting. There are no set dates when applications need to be in.

High school applicants to the Nursing Program at NSCC must follow special admissions guidelines. Questions should be directed to Karen Short, Dean of Allied Health and Public Services at (419) 267-1253 or Marsha Beuhrer, Division Secretary at (419) 267-1246.
 
Immunization Form

This form is only required of students who are beginning their clinical courses. The health requirements listed on the Immunization form must be completed and verified prior to the first clinical experience. These requirements must be presented to the departmental secretary for verification upon admission to the nursing program and annually until graduation.

 
Nursing Honor Code

The education of all nursing students at Northwest State Community College is based on the concept that integrity, responsibility, accountability, and self-discipline are inherent to the profession of nursing. All Nursing students are required to sign and return the Nursing Honor Code at the start of their program.
